NOTICE: This information is only for those Enrolled Program Claimants that are seeking pharmacy records from Express Scripts, Inc. ("ESI"). If you are not seeking pharmacy records from ESI, then this information and these deadlines do not apply.
You received an email notification on February 15, 2008, that the Plaintiffs’ Negotiating Committee, Plaintiffs’ Steering Committee and Liaison Counsel in New Jersey were negotiating with Express Scripts, Inc. ("ESI") to obtain pharmacy records for all claimants who may enroll in the Vioxx Settlement Program. That original message contained an Excel spreadsheet for you to complete and submit to Anne Marie Sweeney (Michael Weinkowitz’ Assistant) at ASweeney@lfsblaw.com as soon as possible.
