On or before July 1, 2008, each Enrolled Claimant must submit a Claims Package, including a Claims Form, required PME Records, and a Plaintiff or Claimant Profile Form (if previously completed and submitted). If a Plaintiff Profile Form or Claimant Profile Form has not previously been submitted, Attachment A to the Claims Form must be completed and signed by the Claimant.
